May 1, 2026 – May 14, 2026

During this time, you may:

• Add coverage
• Drop coverage
• Make changes to your current medical elections

What You Need to Do

• If you wish to make a change, please complete the appropriate form:
• Active Employees: Active Employee Status Change Form
• Retirees: Retiree Status Change Form
• Forms can be found online @ www.myffbenefits.com

Completed forms must be submitted no later than May 14th
