Open Enrollment for Voluntary Supplemental / Life Insurance Benefits

October 1 – November 3, 2025
Effective January 1, 2026

Beginning January 1, 2026, the UnitedHealthcare Supplemental Benefits for Accident, Hospital Indemnity, and Critical Illness will transition from Fund-paid coverage (included in your 2025 Medical premium) to voluntary, self-pay coverage.

These plans provide cash benefits for covered services listed in the Supplemental Benefits Guide and continue to include the annual wellness benefit many members have received for completing their physicals.

To continue your coverage, you must enroll by November 3, 2025.
The enrollment form and supplemental guide are available below.

In December 2025, you will receive a welcome letter outlining your coverage details, monthly premium, and first payment due date.

You will also receive instructions to access the billing portal, where you can view invoices and choose your preferred payment method.
Monthly invoices will be mailed separately.

Payment options include ACH and recurring credit card payments, which are processed on the 5th business day of each month (please allow a few days for the transaction to appear on your bank statement). This consistent schedule helps ensure your payments are made on time and easy to track.
